Subacute thyroiditis: clinical significance of diffuse 18-fluorodeoxyglucose uptake. Case report

Background. Subacute thyroiditis is a rare inflammatory thyroid disease presumably associated with viral infection, including COVID-19. It accounts for about 5% of all diseases.
 Materials and methods. A clinical case de Quervain's subacute in patient diffuse hypermetabolic 18-fluorodeoxyglucose uptake by the gland presented.
 Results. 61-year-old no history disorders 3 weeks after COVID-19 experienced fever evenings (37.839.8C). He had moderate pain radiation to jaw, severe weakness, hand tremors, shivers, 12 kg weight loss, sweating. These symptoms lasted weeks. Physical examination showed an enlarged gland, dense extremely tender, especially its left lobe. Ultrasound heterogeneous structure large hypoechoic areas both lobes up 47 mm fuzzy contour. Color Doppler imaging revealed reduced blood flow. Combined positron-emission X-ray computed tomography increased radiopharmaceutical enlargement lobes, SUVmax 10.55. Blood thyroid-stimulating hormone low, free triiodothyronine thyroxine are high, ferritin concentration markedly increased, consistent thyrotoxicosis. fine-needle aspiration biopsy was performed. Cytology confirmed thyroiditis. Glucocorticoids nonsteroidal anti-inflammatory drugs were administered. Four treatment, manifestations resolved. The assessed 6 months. regular size smooth contours; color mapping normal Thyroid-stimulating hormone, triiodothyronine, within reference ranges.
 Conclusion. SARS-CoV-2 virus can be regarded as cause intensive gland. basis differential diagnostics. first-choice agents.
